There isn't anybody who wants to see themselves or their colleagues get hurt or die at work. Yet, it happens. There is also no shortage of rules, processes, procedures and certification systems in place. Yet, it still happens. 

Often, part of the response to an unfortunate incident is the desire to add more process in an effort to gain added control over the work environment. However, one of the consequences is an expansion of activities that consume more and more time but don't necessary make the impact that was hoped for. Part of the problem is that no amount of process will ever let you control 100% of the work environment, just like you can't completely eliminate risk.
